LOCATION:  

Our office is located in the Sacred Heart Hospital, DePaul Building.  Directions to our office:  enter Sacred Heart Hospital through the DePaul Building Entrance (near Bayou Parking Garage).  Take Elevator G, to the 3rd floor.  Follow the hallway, we are in Suite 322.

Mon.-Thurs. 8:00 AM-5:00 PM                                  
Closed Friday-Sunday

APPOINTMENTS:                 

At each visit you will be given a  follow-up appointment, as indicated.  We make every attempt to contact you via telephone prior to your next appointment, however it is ultimately your responsibility.  If you are late for your appointment, we will make every effort to work you in, however we will see the  patients who were on time first.  If you cannot be present for your scheduled appointment, please contact our office 24-hours in advance to reschedule.  This allows us an opportunity to accomodate other patient needs.  You will be charged a fee of $20.00 if you do not cancel your scheduled appointment with a 24-hour notice.

INSURANCE INFORMATION:          

We accept and file claims for most insurance carriers, however if an authorization is required by your insurance, it is your responsibility to ensure this is obtained prior to your visit.  You are expected to pay your co-payment and deductible at the time of your visit.  We accept cash, check, MasterCard, and VISA. Private pay patients are expected to pay for services at the time of the service.  Alternate payment arrangements may be  made in advance of the appointment.

REFILLS OR NEW PRESCRIPTIONS:                              

Please contact your pharmacy to order a refill medication.  Your pharmacy will contact our office for approval, which may take up to 48 hours.  It is important that you keep this in mind when requesting refills.  Refills will not be provided after hours or on weekends, unless it is an emergency.  An office visit is necessary for new medication prescriptions, as the medical provider must evaluate your current medical symptoms, conditions and current medications to effectively prescribe the most appropriate medication.

PROBLEMS/QUESTIONS:              

Patients are welcome to contact the Practice with any problems, concerns or questions.  In the event someone is not available at the time of your call, you will be directed to leave a voicemail on our telephone system.  All voicemails are checked throughout the day.  Often consulation with the medical practitioner is necessary prior to returning your call. Calling multiple times slows down the process.

FORMS/LETTERS:                            

An appointment is necessary when requesting completion of forms or letters.  The practitioner must fully understand the information being requested, and discuss your current medical condition in order to accurately related the appropriate medical information to the source.  We must also have a medical release of information signed by you, for each particular release of your medical information.  This document may be downloaded from our website, and completed in advance of your appointment. 

HOSPITAL ADMISSIONS/CONSULTATIONS:                           

Our practitioners have privileges at the following facilities:  Sacred Heart Hospital, Select Specialty Hospital, Baptist Hospital, and West Florida Hospital.
