About Us

The Center for Prevention and Treatment of Infections provides comprehensive care through a multi-disciplinary team approach.  By using this approach, each client benefits from the experience and expertise of an entire team of professionals who work closely with the client and one another.  Staff members are able to interact with each client and develop an understanding of the client's unique strengths, needs, concerns, and challenges. Throughout the process, the staff maintains a commitment to client confidentiality.  The team approach provides a powerful knowledge base to provide the client with the best possible health plan.  The team consists of client, physician, physician assistant, nurse, medical assistant, and social worker.

General Information

LOCATION:  

Our office is located in Sacred Heart Hospital.  To get to the office, you can enter through the Main Entrance facing Ninth Avenue.  Take Elevator A (located before Wendy's and Subway) to the 3rd floor.  We are in Suite 305.

OFFICE HOURS:   

Mon.-Thurs. 8:00 AM-5:00 PM                                  
Fri. 8:00 AM-NOON

(note: Office is closed from noon until 1:00 PM for lunch) 

APPOINTMENTS:                 

At each visit you will be given an appointment card for the follow-up visit.  We try to call patients to remind them of upcoming appointments, however it is ultimately the patient's responsibility to keep up with appointments.  You may be charged if you do not cancel 24 hours prior to your appointment. We make every effort to see patients promptly.  If you are late we will try to work you in, however we will see the  patients who were on time first.

INSURANCE INFORMATION:          

We accept and file claims for most insurance carriers, however if an authorization is required by your insurance, we will need it prior to your visit.  You may need to assist us in getting this authorization.   You will be expected to pay your co-payment at the time of your visit.  We      accept cash, check, MasterCard, and VISA. Our office participates in a program that pays for the care of patients with HIV who have no insurance.  For information about this program, please contact one of our   office social workers. Private pay patients are expected to pay for services at the time of the service.  Alternate payment arrangements may be  made in advance of the appointment.

REFILLS OR NEW PRESCRIPTIONS:                              

To order a refill you must contact your pharmacy.  If no refills are remaining, your pharmacy will contact our office.  It may take our office up to 48 hours to approve, therefore it is important that you keep this in mind when requesting refills.  Refills will not be provided after hours or on weekends unless it is an emergency. If you need a new prescription, you will need to schedule an appointment to see the physician to address the new problem.

PROBLEMS/QUESTIONS:              

Patients who have questions will be directed to leave a message on the nurse's voicemail.  The nurse will    listen to the message, consult with the doctor, then will return your phone call.  The nurse will check the voice mail throughout the day and will return your call prior to the close of business each day. Calling multiple times only slows down the process.

FORMS/LETTERS:                            

An appointment is necessary when requesting completion of forms or letters.  The reason for this policy is so that the doctor can understand your request and is able to review your medical  record.

HOSPITAL ADMISSIONS/CONSULTATIONS:                           

Our doctors see patients at Sacred Heart as well as Baptist and Gulf Breeze Hospitals.
