HIV Care Center

The mission of the HIV Care Center, Inc. is to provide services to individuals with HIV/AIDS necessary to assure their optimal health.

Services

  • Medical Care and Lab Work for uninsured individuals with HIV/AIDS
  • Linkage for patients being seen by CPTI
  • Patient Assistance (assistance with free medications available through programs set up through individual pharmaceutical companies)
  • Transportation Assistance for Medical (or medical related) appointments (i.e. bus passes) as available
  • Project AIDS Care (PAC) Medicaid Waiver - providing case management and other needed services to individuals with a diagnosis of AIDS who are at risk of hospitalization
  • Assistance with completing applications for Medicaid, Food Stamps, and Social Security
  • Medical Adherence and Retention in Care (MARC) Program - 16-week program designed to assist individuals in overcoming barriers to medical care
  • Lifestyle Modification Specialist - provides mental health and substance abuse counseling to individuals as needed
  • Adherence Counseling (assist in filling pill boxes for patients)
  • Prevention Education
  • Individual HIV Education
  • Nutritional Supplements (as available)
